Salary: £25000£28000 On call Pay Travel allowance of £1200
On call duties 1 day per week i.e. 6am9am or 5pm10pm) £1000 On Call Allowance
Reporting to: Registered Care Manager
Location: Westminster London
Schedule: MondayFriday
Care setting: Domiciliary Care

Purpose of the role:


my client is a wellestablished care provider in London with over 20 years of experience in providing care to the elderly at home. The are looking for a fantastic field care supervisor to support their business who has experience within the sector.

The role of a Care Coordinator involves working closely with the Local Authority Health care Teams and other providers. The ideal candidate will need to be computer literate and able to pick up new systems relatively quickly and project a positive and professional image at all times assisting home care workers promptly and conducting all business in a friendly courteous and knowledgeable manner.
You will be responsible for on call duties Monday to Friday as rostered by you line manager.

Key Responsibilities:
  • To produce care staff rotas on a weekly basis using the organisations computerbased roster system.
  • All records and rotas to always be up to date and be compliant to the organisational and legal requirements of a safe and wellplanned service.
  • To maintain runs of work that are geographically planned which use resources efficiently and continuously reviewed and updated as necessary.
  • To be responsible for reviewing and accepting processing and allocating referrals with the Field Care Supervisors
  • To compile client information/Home Care Worker management actions and report weekly by utilising Key Performance Indictor (KPI forms to monitor the strategic performance and compliance against each area meeting the standards and targets agreed for a weekly deadline
  • To allocate calls to care workers taking into consideration Service User's needs and preferences for gender and compatibility.
  • To take responsibility for ensuring that all calls are covered in all instances
  • To provide care workers with full briefings about Service User's needs and any other information that is required to deliver a good service. Providing updates as Service User's needs.
  • To inform care workers of any amendments to their schedules as they occur with careful clarification.
  • To ensure accurate records are maintained on the Carefree roster including Service User and staff information a daily event record and details of service delivered staff holidays absence and training.
  • To proactively liaise with Social Workers Service Users Family Members and Care Workers to take on new referrals and arrange with the Care Manager to plan initial assessments.
  • To communicate effectively with Service Users staff colleagues GP's Social Workers and other members of the multidisciplinary team to promote a high standard of care/support. Ensuring information is shared on a needtoknow basis and in the best interest of the Service User.
  • To ensure confidentiality of Service User information in line with the Organisation's policy procedures.
  • To report any incidents or complaints to their line manager completing incident report forms and other documentation as required.
  • To undertake on call duties as part of an agreed preplanned rota in order to provide an effective out of hours oncall service.
  • To work closely with the team to plan in a proactive manner.
  • To monitor the performance of care workers organising spot checks and conducting supervisions and appraisals in line with the Organisation's policy on keeping accurate records.
  • To have knowledge of all relevant policies and procedures in line with CQC and be familiar with processes for communication and reporting.
  • To routinely identify and report to your line manager any areas of concern potential business development opportunities and quality compliance related issues

Qualifications:
  • NVQ level 2
  • Willing to work towards an NVQ level 3
  • Willing to work towards supervision and management qualifications as appropriate

Essential Experience:
  • A positive attitude as a creative and innovative problem solver with excellent communication skills and be able to identify and describe problems in high detail.
  • Professional while dealing with a variety of staff both internally and externally
  • A patient understanding clear communicator who is able to remain professional under pressure and to deal with challenging situations in a sensitive and creative manner
  • Experience with using a roster within Domiciliary Care
  • Excellent problemsolving skill
  • Willingness to work flexibly and ability/commitment to work as part of a team and on your own initiative
  • Flexible approach to working hours (with ability to work hours to needs of the business)
  • Be competent using computer packages such as Microsoft Office Carefree Roadrunner Birdie Home Care CM2000 or EziTracker applications
  • A good team ethic
  • Working knowledge of regulatory frameworks (CQC)
  • Working Knowledge of Health and Safety requirements

Desirable Experience:
  • Health & Social Care experience (paid or voluntary)
  • Level 2 or 3 in health and social care
  • Experience of call centre style environment
  • Supportive – recognise how to contribute to the team to help
