Mental Health Practitioner Job Requiremnets We are hiring a Mental Health Practitioner to work as part of the Community team. Your role includes; To be able to undertake triage and access assessments and risk assessments. Should possess knowledge of local areas/services and signposting following assessment to meet assessed needs.
