Pharmacist







Our client is now looking for a PCN Clinical & Community Pharmacist who is enthusiastic, highly motivated, and above all passionate about making a difference. They need someone with a proven track record and ideally a minimum of 2 years experience. You will be based at large surgery and on site pharmacy in Peterborough located in the heart of the community.
The role involves collaborative working with the on site pharmacy and the surgery. It will involve working between both the surgery and the pharmacy. Typically this will be 3 days at the pharmacy and 2 days in the surgery. (Mon-Fri)
They will provide full training and this will include training to become an IP pharmacist.
They need someone willing and able to take command and lead the team to the enhance current services as well as develop new and innovative services.
Tasks
KEY DUTIES AND RESPONSIBILITIES OF THE ROLE IN THE PRACTICE:
Undertake and record reviews of discharge/clinical letters where medication is involved to ensure all changes are accurately recorded on the clinical system and patients contacted where changes to medication have occurred or further tests are required
Meet with patients to discuss their medications and consider improving their care by altering medication.
Undertake and record medication reviews with direct patient contact for patients on complex medicine regimes.
Proactively liaise with clinical staff to ensure this service is offered to patients who would benefit from having their medicines optimised
Improve management of medicines to help reduce adverse effects to medicines.
Maximise clinical outcomes for patients by focusing on patient engagement through medication reviews.
Reduce potential waste medicines by medicine synchronisation and identifying non-adherent patients.
Ensure cost effective use of investment in prescribing by ensuring full utilisation of resources and implementation of change to current prescribing.
Undertake medication related audits
KEY DUTIES AND RESPONSIBILITIES OF THE ROLE IN COMMUNITY PHARMACY:
Oversee the dispensing process when on duty
Set up and deliver all the pharmacy services needed including NMS, DMS, Flu, CPCS, BP checks.
Offer specialist health checks, such as blood pressure and cholesterol monitoring and diabetes screening
Run stop-smoking clinics and weight-reduction programmes
Liaise with surgery regarding all prescription interventions
Advise the public on the treatment of minor ailments
Train the team of 8 dispensers to deliver their role
They have an ACT Pharmacy Operations Manager who will manage all the day to day pharmacy operational activities and all the HR matters so you can focus on patient care and pharmacy services.
