We are hiring a Mental Health Practitioner to work as part of the Community team.
Your role includes;
- To be able to undertake triage and access assessments and risk assessments.
- Should possess knowledge of local areas/services and signposting following assessment to meet assessed needs.
- Should have experience in short-term casework up to 3 months to monitor risk, recovery, and medication monitoring.
